Forms for individual requests (private parties) can be downloaded here: FAC001-individual use this one.doc At this time we do not rent indoor facilities for private parties. A copy of homeowners insurance must be submitted with your facility use request. Forms for organization requests can be downloaded here: FAC002-USE THIS ONE.doc A certificate of liability insurance must be submitted with your facility use request.
